Neurogenic Thoracic Outlet Syndrome occurs when nerves in the space between your clavicle (collarbone) and your thoracic outlet (first rib) are compressed.

The result can be pain in your shoulder and/or neck and numbness in your fingers.

Neurogenic Thoracic Outlet Syndrome is characterized by the compression of the lower part of the brachial plexus, a network of nerves that comes from your spinal cord and controls muscle movements and sensations in your shoulder, arm and hand.

Symptoms of Thoracic Outlet Syndrome

  • The signs that your nerve structures in your brachial plexus are compressed include:
  • The loss of muscle mass in the base of your thumb
  • Tingling or numbness in your arm or fingers
  • Aches and pain in your neck, shoulder or hand
  • An increasingly weak grip

While the compression of nerves is the root of neurogenic Thoracic Outlet Syndrome, the causes of the compression can vary:

  • Trauma: such as a car accident. Can have a delayed onset.
  • Pressure on Joints: due to being overweight or an overloaded purse or backpack
  • Pregnancy: your joints loosen naturally when pregnant which can lead to the appearance of thoracic outlet syndrome symptoms
  • Repetitive Stress Injury: work or sports activities that involve repetitive motions such as stocking shelves, pitching or swimming can elicit thoracic outlet symptoms
  • Bad Posture: allowing shoulders to slump forward or holding your head forward can compress your thoracic outlet area
  • Anatomical defect: congenital defects (inherited at birth) including an extra rib or an unusually tight fibrous band connecting your spine to your rib can cause thoracic outlet symptoms

Testing for Neurogenic Thoracic Outlet Syndrome

In addition to a complete physical exam and a review of previously administered tests and their results your doctor may refer you for an Electromyography (EMG) or a Nerve Conduction Study (NCS). The results of these tests may help establish a more accurate diagnosis and treatment plan:

  • EMG – Tests the electrical activity of the muscles to determine if they suffered loss of nerve supply.
  • NCS – Nerve conduction studies (to evaluate the function of the motor and sensory nerves)
